Please read all instructions below to ensure that all requirements are met. Failure to follow the below instructions will result in a loss of filming privileges.

This will act as your conditional filming permit during your production period. It must be filled out by your chosen dedicated group member for on-campus projects; including in-class lab time and MEA facility studio usage.

Students must: 

– Scout the location.

– Have a group member print the form.

– Bring the form to their instructor to approve.

– Turn the approved form in to MEA Staff in Mentry 325 during open hours to be filed. 

NOTE: This is only a location request. The class instructor must approve of this form before occupying the spaces during dates and times you specify below. Failure to complete this form will reject group’s access to equipment and/or filming.
